OLYMPIA – The Washington State Department of Health has imposed sanctions or sanctions on the following health care providers in our state.
The Department of Health Systems Quality Assurance Division works with boards, commissions, and steering committees to limit the licensing of more than 80 health workers (for example, dentists, nurses, counselors). Clallam County
Information about health care providers is available on the emergency website. Click on “Apply for a health care license” in “What to do?” part of the website of the Department of Health (doh.wa.gov). This site includes information on the quality of health care services, expiration date and date of update of their records, disciplinary actions and copies of legal documents published after July 1998. This information is also available by calling 360-236-4700. Consumers who feel that the healthcare provider has acted unfairly are encouraged to call and voice their complaint.
King County
In May 2022 the Agency Affiliated Counselor Program and the Substance Use Disorder Professional Program hired an agency affiliated with counseling and substance abuse trainee trainee Daniel Thomas Clemons (CG61163703, CO61198606) and a nonprofit. In May 2022 the Marriage and Family Therapy Program accused marriage and family therapist Linda Lee Hanby (LF60256554) of irresponsible behavior. While helping the teenage girl’s father, Hanby allegedly threatened the boy’s mother with a possible life imprisonment if she did not force her daughter to take part in the reunion. He allegedly encouraged the mother to lie to the teenager as it would cause him harm that could cause injury or harm to the child.
Pierce County
In May 2022 Pharmacy Commission charged medical assistant Torben Suhr Heinecke (VB60860433, VA60986270) with non-compliant procedures. Snohomish County

In May 2022 the Massage Therapist Program charged massage therapist Dan Guo (MA60322250) with irresponsible behavior. Two reviews of Goo’s practices are said to have shown health, security, record keeping, and marketing breach.
In May 2022 the health secretary was granted the status of a joint licensing agency by John Herlin Tollefson (CG61192082). Tollefson’s license will be in place for two years. Also, he should have a specialist who oversees his performance and submits a quarterly review. In 2019, Tollefson was found guilty of providing assistance to the Spokane County Superior Court.
In May 2022 the Board of Osteopathic Medicine and Surgery accused an osteopathic physician and surgeon and former Pacific Northwest University professor Larry Sherman Lefors (OP00000767) of unethical behavior and claims to students. Lefors allegedly rubbed another student’s leg, pressed his body against the student’s back, and talked about the size of the student’s breast, among other things.
Minnesota: In May 2022 the Dental Commission accused dentist Michael C. Johnson (GA60673242, DE60642383) of negligent conduct. Johnson allegedly improperly implanted implants, improperly extracted teeth but failed to notify the patient’s caregivers, and cut the patient’s nerves, among other things.
