OLYMPIA — The Washington State Department of Health has revoked or suspended the licenses, certifications or registrations of the following health care providers in our state. The department has immediately suspended the credentials of individuals who are prohibited from practicing in other states.
The Department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory boards to establish licensure standards for more than 80 health professions (eg, dentists, nurses, counselors). Information on disciplinary actions taken against physicians and medical assistants can be found on the Washington Medical Commission (WMC) website. Questions regarding WMC disciplinary actions may be directed to media@wmc.wa.gov.
Cowlitz County
Information about health care providers is available on the agency‘s website. In the “Find a health care license” section, click “How do I?” Department of Health home page section (doh.wa.gov). The site includes information on a health care provider’s licensure status, credential expiration and renewal dates, disciplinary actions, and copies of legal documents issued since July 1998. This information is also available by calling 360-236-4700. Consumers who believe a health care provider has acted unprofessionally are also encouraged to call and file a complaint.
Franklin County
In June 2022, the Dental Board suspended the dental assistant license of Mandy Shanee Crane (D161206246) for failure to comply with the requirements of a monitoring program.
Grant County
In June 2022, the Nursing Assistant Program and certified nursing assistant Shaelynn Marie Logozzo (NC60855746) agreed to suspend her license. Logozzo was convicted of two counts of assault in Benton County District Court.
King County
In June 2022 the Health Secretary Alison Dawn Sedlacek (HM60620759) suspended home support worker. On the same subject : A state committee is holding a hearing on youth mental health. Sedlacek told his employer he took a patient’s hydrocodone pill and did not respond to an investigator about the allegation.
In June 2022, the Board of Nursing suspended the licensed nurse of Corina Saldivar (RN60474579) after she withdrew injectable hydromorphone from several patients, but failed to document the administration or waste of the medication.
In June 2022, the Substance Use Disorder Program and Stephen Robert Adkins (CP60264602), a chemical dependency professional (now Substance Use Disorder Professional), agreed to a license suspension order. Adkins disclosed personal and emotional problems to a patient, which caused the patient emotional distress and threatened recovery.
In June 2022 the Secretary of Health suspended the licensed physician assistant of Heidi Marie Vaughn (CM60366750). Vaughn was convicted of aggravated robbery in Federal Way Municipal Court and Snohomish County District Court in 2015. In 2016, Vaughn was convicted of unlawful solicitation to possess a controlled substance in Pierce County Superior Court. Vaughn did not respond to the charges surrounding those judgments.
Kitsap County
In June 2022, the health secretary suspended Angela Lorraine Walker’s (NC60508680) certified nursing assistant for at least two years. Walker was charged in Kirkland Municipal Court with attempted identity theft and attempted theft after allegedly making unauthorized purchases with a patient’s debit card to pay rent.
Pierce County
In June 2022, the health secretary suspended the mental health counselor license of David Scott Secrest (MC60842091) for at least two years for having a romantic relationship with a patient and failing to keep records of his sessions.
In June 2022 the Secretary of Health suspended the registered nursing assistant license of Alexandra Smith-Rosas (NA61172518). Smith-Rosas exhibited behavior at work that appeared to be under the influence of an intoxicant. He told an officer he had taken Percocet and cocaine. Smith-Rosas did not respond to the allegations.
Out of State
In June 2022 the Secretary of Health suspended Tony Mandel Johnson’s radiologic technologist license (RT60040761) for at least 10 years. Johnson was convicted of two counts of child abuse in Pierce County Superior Court.
Montana: In June 2022, the Board of Nursing suspended the registered nurse license of Kathryn S. Fowler (RN61015330) after the Montana Board of Nursing agreed to relinquish Fowler’s credential after finding that Fowler engaged in unprofessional conduct when she reduced the intravenous fluids of a pediatric patient. without a doctor’s order and without contacting the doctor.
New Mexico: In June 2022 the Board of Physical Therapy suspended the physical therapy assistant license of Daniel Kuemmerle (P160889931). In an agreed order in Minnesota, he admitted to unprofessional conduct by intentionally increasing his time with clients and billing when work was not done. He did not respond to charges in Washington state related to the conduct in Minnesota.
