A Request for Applications (RFA) is used to award grants to promote specific program goals. An organization (in this case, the Colorado Department of Public Health and Environment) announces that a grant is available, and eligible organizations can apply. Funding under RFA #40624 specifically supports the following goals:

Expand and/or develop new intervention and prevention tools and services to reduce the disparity associated with COVID-19 among disproportionately affected populations.

Build, leverage, and expand support against COVID-19.

Advance health equity and address social determinants of health as they relate to health disparities in COVID-19.

Increase COVID-19 vaccine capacity in all regions, including high-risk and underserved populations.

Ensure high-quality and safe administration of COVID-19 vaccines.

Ensure equitable distribution and administration of the COVID-19 vaccine.

Increase vaccine confidence through education, awareness, and collaboration.

Develop and implement community engagement strategies to promote COVID-19 vaccination efforts.

Support higher vaccination uptake among tribal nations.

There are 4 program areas to support funding goals. Applicants may apply for more than one program location. See the RFA for more information and detailed instructions. There is no fixed award amount or maximum award; Please submit a funding request that is appropriate for your agency’s needs.

This program will fund projects/activities that use authentic community engagement practices to gather community perspectives on the public’s unique assets and needs related to health equity and the COVID-19 disaster.

This program will provide support to community health leaders to carry out vaccination awareness, patient education, and health procedures.

This program will fund activities focused on community resilience related to social determinants of health as they relate to the impact of COVID-19 on communities.

The mission of this program is to improve vaccine education, awareness, access, and management in communities.

Applicants must be based in Colorado.

Eligible organizations include but are not limited to:

Non-profit organizations, in good standing with the IRS and the State of Colorado

Community-based and grassroots organizations

A federally recognized Native American tribe in Colorado, or a non-profit organization that provides services to a Tribe entitled to a reservation or federally recognized Tribal land with a letter of support from the applicable Tribal council

Community health centers/rural health centers/safety net centers/federal health centers

Collection sites: nursing and assisted living facilities, prisons/jails, shelters, etc.

Educational institutions: K-12 schools in the program (research is not eligible)

Government agencies: city councils, city departments and municipalities, and Local Public Health Agencies (LPHAs)

(Note: Agencies that have already received funding from CDPHE or CDC for the same work are not eligible to apply. LPHAs interested in receiving funds under this program must demonstrate that other available COVID-19 assistance funds do not cover the proposed activities. said.)

Organizations with financial sponsorship of 501(c)(3)

Historically underserved minority communities and communities of color;

People with socioeconomic status;

Lesbian, gay, transgender, genderqueer, or queer individuals;

Gender-queer, non-binary, and other gender-nonconforming individuals;

Members of immigrant and refugee communities;

People experiencing homelessness;

Community-based organizations and groups;

Rural and/or geographically isolated;

Programs, services, and/or events that are culturally and linguistically responsive;

Organizations that engage with the target community to inform them of the programs, services, and/or events to be offered and/or engage in leadership roles in the organization.
