OLYMPIA – The Washington State Department of Health has taken disciplinary action or withdrawn charges against the following health care providers in our state.
The health systems quality assurance division of the department works with boards, commissions and advisory committees to set licensing standards for more than 80 health professions (eg dentists, nurses, consultants). Information on disciplinary action against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary action can be sent to media@wmc.wa.gov.
Benton County
Information about healthcare providers is available on the agency’s website. Click on “Check a healthcare provider’s license” in the “How do I do it?” Section of the Department of Health website (doh.wa.gov). The Site contains information about the license status of a health service provider, the expiration and renewal date of its registration information, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who feel that a healthcare provider has acted unprofessionally are encouraged to call and report their complaint.
Chelan County
In June 2022, the Board of Physical Therapy accused physiotherapist Amy Lynn Lewis, formerly known as Amy Lynn Stone (PT60902515), of unprofessional conduct. Lewis allegedly failed to comply with a substance abuse monitoring program required as a condition of her license.
Clark County
In June 2022, the Board of Directors of Optometry agreed to conditionally reactivate the optometrist license of Lisa Lachelle Robinson (OD00003122). Robinson, who agreed to the terms, will have to put his credentials to the test for at least three years, must practice with a mentor, and receive or receive further advice, among other things. In May 2022, the Secretary of State rejected Jay Joseph Cesart’s relief application Remigio Tionquiao (HM61232864). Tionquiao was charged with theft in Kitsap County Superior Court. Under state law, a care worker with certain pending costs is disqualified from working with vulnerable people.
In May 2022, the Exam Board of Psychology finalized the terms on the license of psychologist Philip J. Frank (PY00000510), although his practice is now permanently excluded, given court-ordered therapy or treatment, reconciliation therapy, and parental evaluations.

In June 2022, the Substance Use Disorder program charged the substance abuse professional Fleet Montgomery Daly (CP60890141) with unprofessional conduct. Daly admitted to using cocaine, a controlled substance and allegedly grabbed his girlfriend by the neck, beating her and beating her, resulting in bodily harm. He was arrested for assassination in October 2021.
In June 2022, Social Workers and Nursing Assistant Programs prosecuted independent clinical social workers and certified Nursing Assistant Rhaejon Monique Ongers (LW00009729, NC60951019) with unprofessional conduct. The Ungers allegedly attended a work meeting with bloody eyes, impaired speech, and irregular movements and a test allegedly showed that they had high blood alcohol levels.
In June 2022, the Home Care Aid Program accused the Home Care Aide Anna Kay Leamon (HM60814602) of unprofessional conduct. Leamon allegedly received a $ 1,200 gift from a patient in whose home she lived. Leamon also allegedly shared medical information of a patient with an unrelated third party.
