OLYMPIA – The Washington State Department of Health has revoked or suspended the licenses, certificates, or registration of incoming health care providers in our state. The ministry also immediately suspended the certificates of people banned from working in some states.
The Health Quality Assurance Department works with boards, committees and advisory committees to set licensing requirements for more than 80 healthcare professionals (e.g., dentists, nurses, nurses propose). Information on disciplinary action against physicians and assistant physicians can be found on the Washington Medical Center (WMC) website. Snohomish County
Information about health workers is available on the agency’s website. Click on “Apply for a driver’s license” in the “How do I do it?” section of the Department of Health’s home page (doh.wa.gov). The site contains information on the status of the health worker’s license, the expiration date and renewal of their certificates, disciplinary actions and a copy of the legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health worker has not worked are also encouraged and are encouraged to call and report their complaints.
Stevens County
In May 2022 the Dental Board revoked Gene Melner’s dentist’s license (DN60457532). A patient with dental implants at Melner Hospital went to pick them up and found the office was closed. Melner did not provide any information to investigators when contacted about the allegations.
Out of State
In May 2022 the Assistant Nurses Program and Jamie Jo Sillett (NC61188089, NA61040631) entered into an agreement to suspend Sillett’s registered nursing staff. Sillett takes most of the patient’s oxycodone pills.
Montana: In May 2022 the Health Care Authority revoked the license of the registered nurse Kelley Morrow Koch (RN00087640) after the Montana State Medical Board revoked the certificate of the Koch nurse.
